If this is your first appointment, please submit the form at the bottom of this page, acknowledging that you understand all twelve statements.  There is important disclosure information as well as information that will help us form a solid working relationship.

Disclosure Statement:

  1. Ethics:  I follow the ethical standards and guidelines of the American Counseling Association (ACA) and the National Board of Certified Counselors (NBCC) for online counseling.  I am licensed by the State of Illinois.

  2. Age:  Clients must be at least 18 to use this service or have written permission from parents or guardians.

  3. Exclusion:  While online counseling is effective for many, it cannot substitute for face-to-face counseling in some circumstances.  I will refer you elsewhere if appropriate.

  4. Confidentiality:  Your personal information is encrypted and sent to me via a secure server.  All information is stored securely.  It is kept confidential unless you provide a signed release of records to a third party. I make every recommended effort of the ACA and NBCC to mitigate risk of potential breaches of confidentiality.

  5. Confidentiality Limits:  Complete confidentiality over the Internet or telephone is not possible, since both can be intercepted by third parties despite all reasonable efforts to arrange a secure online environment. Counseling ethics mandate that confidentiality must be broken when 1) a client is suicidal or homicidal, or a threat to themselves or others, 2) there is indication of child or elder abuse, or 3) a therapist is sued or subpoenaed by court order. 

Working Agreements

  1. Emergency: If you are feeling suicidal or homicidal, or are in need of immediate help and cannot reach me, please dial 911 or a help line in your area.

  2. Technological Failure: If we become disconnected, I will call you back.  You will be credited with  unused time.

  3. E-mails: Are checked every 12 hours.

  4. Time Zone: My appointment calendar is formatted in the Central Time Zone (Chicago).  Please take that in to account and synchronize your appointments to Central Time.

  5. Payments: All sessions are prepaid prior to the appointment.  If you cancel more than 24 hours in advance of an appointment, fees will be fully refunded.  If you miss an appointment, 50% of the fee will be refunded.

  6. Insurance:  Internet and telephone counseling is not reimbursed by most insurance companies.  If you do wish to submit a claim to your company, I will forward all necessary paperwork to you.

  7. Telephone:  I will call you at the appointed time.  You will not have to pay long-distance charges.
